Step 1: Create an online account to place an order for CLIA Waiver Assistance.

Step 2: You’ll receive a confirmation email with a link to book a 15-minute virtual meeting with us. During this meeting, we will ask you the necessary information to fill out the CMS-116 form. 

Step 3: After our meeting, we’ll send you the completed form to sign via Docusign. SiteLabs will send your form to the state agency. 

Step 4: Pay the certificate waiver fee. We are available to answer any questions regarding the payment process.
